I originally approached this section believing that a good toothpaste should contain a remineralizing ingredient, and I still believe supporting remineralization is vital. But I also believe the conversation is much bigger than choosing the “right” tube of toothpaste.
In an ideal world, we would all have healthy saliva, a balanced oral environment, great home-care habits, and very few outside factors working against our teeth. That simply isn’t reality for most people.
In practice, I see people sipping and snacking throughout the day, missing the gumline when they brush, rarely cleaning between their teeth, struggling with dry mouth or mouth breathing, and using multiple products that may alter the oral environment. All of these things matter.
Fluoride and hydroxyapatite are two of the most common remineralizing ingredients discussed today, and there is research supporting their use. There are also questions and concerns that lead some people to prefer one over the other. Our understanding of these ingredients, the oral microbiome, and remineralization continues to evolve.
Beyond that, I believe simpler ingredient lists are better when possible. I see many patients, myself included, experience irritation from ingredients such as SLS (sodium lauryl sulfate). It may contribute to mouth irritation, including sores and tissue sloughing, for some individuals.
At the same time, I don't believe we should become consumed by fear or stress over every ingredient, and I mean that on both sides of the debate. Do your research, ask questions, and talk with your dental professional about your specific risk factors. Then choose products that align with your personal needs, values, and stage of life.
So what do I recommend?
Start with the things we know matter tremendously: good brushing technique, cleaning between the teeth, consistency with home care, reducing frequent sipping and snacking, and supporting healthy saliva. Toothpaste can help, but it cannot compensate for everything else.
And be willing to change your mind.
Research advances. Products change. New information continues to emerge. I would rather be transparent about that than pretend there is one perfect toothpaste for everyone. My goal with the recommendations below is simply to help you understand your options so you can make an informed choice for yourself and your family.
